About Us

We provide specialized Outpatient Mental Health Services.

Our overall goal at Community Wellness Foundation is to provide specialized outpatient mental health services and increase the quality of life through community integration. Our services are tailored to individual needs. Our programs provide community-based comprehensive rehabilitation and recovery services, supports, and promotes successful community integration and use of community resources. We focus on helping individuals develop skills and access resources needed to increase their capacity to be successful and satisfied in the living, working, learning and social environments of their choice. Our approaches are evidence-based best and promising practices in the key life domains of Employment, Education, Leisure, Wellness and Basic Living Skills as well as Family Involvement and Peer Support. Our services are provided in a variety of delivery methods On-Site and Off-Site.

Our Services

Outpatient Mental Health Center (OMHC) – Adults and Minors

Our Outpatient mental health treatment is not one-size-fits-all. Services are offered in home, in school, in any safe and private setting, or in our clinic. Each Client has an individualized treatment plan designed for him or her. We offer therapy and medication management to clients who are experiencing mental health symptoms. Our multi-disciplinary team is comprised of psychiatrists, licensed social workers, psychiatric nurse practitioners, licensed professional counselors, marriage and family licensed therapists and other mental health professionals.

Diagnostic Evaluation

All Community Wellness Foundation (CWC) Outpatient clients receive a diagnostic evaluation by a Board-Certified Psychiatrist, Psychologist, or a Clinical Social Worker; all of whom are licensed and trained to complete such evaluations. The face-to-face screening assesses the individual’s strengths, available resources, and treatment needs.

Individual Treatment Plan (ITP)

A treatment plan is developed in collaboration with the individual and parent/and or guardian. ITP includes the individual’s diagnosis, presenting problems, needs, strengths, treatment expectations and responsibilities

  • Long term and short-term goals in measurable terms and target dates for each goal
  • Criteria for successful completion of treatment and reasons for continuing treatment. A crisis response plan and baseline/progress on objectives

Individual and Family Therapy

Therapy generally occurs weekly until sufficient progress has occurred to decrease the frequency of sessions. Anger and stress management, coping and problem-solving skills, social skills, improved self-esteem, and self-control, improved familial relationships, and behavior management are among some of the common treatment goals worked on within the therapy sessions. These services are provided by licensed professionals such as Clinical Therapists and Social Workers.

Community Integration

Community Wellness Foundation (CWC) goal is to teach emotional, cognitive, and social skills that help those diagnosed with mental illness live and work in their communities as independently as possible. Our program is designed to help participants optimize their personal, social, and vocational competency to live successfully in the community. Activities are determined by the needs of the persons served. Our clients are active partners in all aspects of these programs. Community integration provides opportunities for the community participation of the persons served. The scope of these services is based on the identified needs and desires of the client.

  • Access and utilization of public transportation
